  • When Filing a Case:
    • Visitors must have a valid ID and must pass through a security screening before being granted entry to the courthouse.
    • Electronic devices, including cellular phones, laptop computers, and electronic tablets are prohibited in most federal courthouses.
    • To file a case, you must also show valid photo ID at the intake counter per Amended Admin Order, 2015-4.
    • The Clerk's Office hours are Monday-Friday, 8:30am-4:00pm, excluding federal holidays.

Checklist for Individual New Case - Minimum Requirements:

  • Voluntary Petition B101


    The original is needed for the Court.

  • Credit Counseling Certificate (from approved agency)
  • Photo Identification
  • Your Statement About your Social Security Number B121.


    List your full social security number on the form.

    Schedule of Fees

    Payment must be made in the form of a money order, cashier's check, (no personal checks accepted) or if filing in person. The Clerk's intake office cannot make change, nor can they accept foreign currency.

  • Requesting Installment Payments?


    Application for Individuals to Pay the Filing Fee in Installments B103-A
    If the full filing fee cannot be paid at the time of filing the petition, an application can be filed with the petition requesting the fee be paid in installments. Bankruptcy Rule 1006(b) limits the number of installments to 4 and the final payment must be made within 120 days after the filing of the petition. Please note: The Court requires the application, not the proposed order.

    Under certain circumstances, a request can be made to waive the filing fee. You must meet the qualifications under 28 USC 1930(f) in order to obtain a waiver of the filing fees. Review these qualifications before filing this application. Please note: The Court requires the application, not the proposed order.
